Personal Trainer Credit Card Form

All personal trainers using TW’s facility must keep an active credit card on file. Training sessions will be charged each Friday for the previous week if they have not been paid on day of use. All 1 hour sessions are $15 per session*. Clients are not permitted to come early or stay after their scheduled session with their trainer. Trainers must make appointments to use the gym. Trainers will be charged if you do not cancel your session in advance of scheduled session by text to Laura. I agree to the above Two Worlds Policies for personal training.
